Job summary

Sun River Health is seeking a full-time Health Coach to provide self-management education and support for patients with chronic medical and behavioral health conditions. The position is primarily remote but may require travel to local SRH locations for trainings and meetings as needed.

You will conduct sessions in person or via tele-video in accordance with NYS regulations, develop care plans, and coordinate with a range of health providers.

Qualifications

• High School Diploma / GED required
• Bachelor's Degree is preferred
• ECW experience preferred
• 2+ years healthcare experience preferred

Responsibilities

• Outreaches and engages high risk patients regarding engagement with primary care.
• Facilitates completion of self-management needs assessment with high-risk patients, including social determinants of health screening.
• Develops a self-management care plan with patients; sessions in person or via tele-video per NYS regulations.
• Implements care plans with educational sessions; sessions in person or via tele-video.
• Ensures providers have approved and signed all self-management plans.
• Provides linkage and referral to ongoing service providers, including mental health and medical specialists.
• Documents notes and care plans in the EMR (eCW) timely and accurately.
• Tracks patient progress and maintains detailed case notes in the EMR.
• Maintains ongoing engagement with patients at least twice a year.
• Performs other duties as assigned.

The Health Coach provides self-management education and support to patients with chronic medical and behavioral health conditions, including addressing social determinants of health concerns.

Essential Functions

• Outreaches and engages high risk patients regarding their engagement with primary care.
• Facilitates completion of self-management needs assessment with high-risk patients, including the social determinants of health screening tool.
• Develops a self-management care plan and goals with patients. Sessions are conducted in person or via tele-video (in accordance with NYS regulations). Provides each patient with a copy of their approved plan.
• Implements self-management care plan with educational session(s). Sessions are conducted in person or via tele-video (in accordance with NYS regulations).
• Ensure providers have approved and signed all self-management plans.
• Provides linkage, coordination with, referral to and follow-up with appropriate ongoing service providers, including mental health and medical specialists, utilizing Unite Us and Referral Point.
• Documents notes and care plans timely, and with accuracy, in agency EMR (eCW).
• Tracks/ monitors patient progress and produces/maintains detailed, accurate and timely case notes documented in agency EMR.
• Provides ongoing engagement with patients, at a minimum of two times per year.
• Other duties as assigned.

Required Education And Skills

• High School Diploma / GED required
• Bachelors Degree is preferred

Required Experience

• 2+ years relevant experience in healthcare is preferred
• ECW experience preferred
