Job summary

Sun River Health is seeking a full-time Wellness Liaison in the Poughkeepsie area to coordinate intake and enrollment into Health Home and SRH service lines. The role involves outreach at community sites, health fairs, and events, building strong connections with clients and partner organizations.

The position requires 2-4 years in customer service, a High School Diploma or GED (Bachelor preferred), and strong data-entry skills. A $1500 sign-on bonus is offered.

Qualifications

• High School Diploma or GED required with four years in customer service in appropriate service industries.
• Bachelor's degree preferred.
• 2-4 years of relevant experience.

Responsibilities

• Outreach and education to coordinate intake into Health Home and SRH services.
• Serve as liaison between clients, providers and account management staff.
• Provide outbound reminder calls and assist clients in securing SRH appointments.
• Enter data accurately into required databases (eCW, TREAT, Salesforce-CRM).
• Attend community events and health fairs to promote SRH services.
