Job summary

Sun River Health is seeking a full-time Community Outreach Coordinator I for our Amenia site. The role focuses on developing long-term relationships with external referral sources and coordinating with internal HRHCare programs to improve access to services for at‑risk clients.

The coordinator will manage 15–25 organizational relationships, conduct about 40 visits per month, and report to the Regional Manager of Community Engagement. Evening/weekend work may be required.

Qualifications

• Minimum education: High School diploma or GED; Bachelor's degree preferred.
• 2–4 years of related experience in outreach, community engagement, or health access.
• Strong communication and relationship-building skills to connect with external partners.

Responsibilities

• Manage a geographic territory and map referral channels.
• Develop a portfolio of 15–25 organizations and cultivate relationships at all staff levels.
• Meet visit targets (40 per month) across primary care, women's health, pediatrics, dental, mental health, and HIV/AIDS services.
• Plan effective pre-call and post-call activities and analyze outcomes.
• Build relationships with external referral channels and internal programs; act as liaison between them.
• Track outreach activities in Salesforce or other tracking tools.
• Create and implement marketing strategies to expand the customer base.
• Assist clients with linkage to intake and assessment services and health home intake days.
• Participate in community engagements and team meetings; develop annual account plans.
