Job summary

Reema Health is seeking a Community Health Navigator (Community Guide) in the Edmonds, WA area to build relationships with members, guide them toward engagement in their own care, and help address social barriers using Reema's technology platform.

The role requires Monday–Friday availability, 25–50% travel in a 30‑mile area, remote/field based work, and duties include outreach, caseload management (80‑120 members), coordinating referrals, and documenting care plans while maintaining HIPAA.

Qualifications

• Strong written and verbal communication skills.
• Comfort with learning and using new tools on mobile and desktop.
• High motivation to stay productive on a remote caseload with minimal supervision.
• Ability to collaborate and foster team connections in a remote environment.

Responsibilities

• Conduct phone and in-person outreach and enroll members in Reema's care model.
• Build and manage a caseload (80-120 members) with coverage for others during spikes.
• Assess member needs, including social determinants of health, to build a member-centered care plan.
• Establish trusting relationships and follow through in a timely manner.
• Help members understand why health activities matter and follow through to completion.
• Coordinate referrals and support appointment scheduling and attendance.
• Independently organize workload to hit monthly goals and daily metrics.
• Engage with members via phone, text, email, and in-person visits.
• Connect members to community and health resources, and transport or accompany as needed.
• Maintain confidentiality and document all work in the Reema App.
