Job summary

Community HealthWorks seeks a Community Health Worker to support Medi-Cal members in ECM, focusing on those transitioning from incarceration to the community. The role emphasizes post-release engagement, care coordination, and practical help to stabilize health and housing needs.

You will work across clinics, shelters, and community sites, building trust, updating care plans, and connecting members with essential services while documenting progress and outcomes.

Qualifications

• High school diploma or GED required.
• Strong communication and relationship-building skills.
• Ability to work independently in community settings.
• Knowledge of Medi-Cal/CalAIM is helpful.
• Valid California driver’s license is required or preferred.

Responsibilities

• Engage Medi-Cal members through in-person and telephone outreach.
• Build trust with members to identify goals and next steps.
• Provide post-release engagement and reentry support.
• Complete warm handoffs for ECM services and coordinate care.
• Review care plans and coordinate referrals following release.
