Job summary

WestCare Foundation is seeking a Medi-Cal Assessment Coordinator to ensure timely assessment, enrollment assistance, and connection to Medi-Cal and healthcare benefits for participants reentering the community.

The role serves as a liaison among participants, MCPs, county behavioral health, providers, and community partners to support continuity of medical and behavioral health services after release.

Qualifications

• Minimum two years of Medi-Cal eligibility or healthcare navigation experience.
• Bachelor’s degree in Social Work, Public Health, Healthcare Administration, Counseling or related field preferred.
• High school diploma or equivalent required.
• Ability to obtain/maintain clearance from CDCR as applicable.
• Valid driver’s license and ability to obtain WestCare vehicle insurance.

Responsibilities

• Conduct assessments to identify healthcare, behavioral health, and Medi‑Cal needs.
• Develop healthcare access plans for successful reentry.
• Coordinate access to medical, behavioral health, and substance use services.
• Assist with Medi‑Cal enrollment, reinstatement, renewal, and verification.
• Coordinate plan selection, primary care assignment, and referrals.
• Maintain documentation per HIPAA and organizational requirements.
• Collaborate with CDCR staff, MCPs, and community partners to ensure continuity of care.
• Participate in multidisciplinary case reviews and support treatment goals.
• Educate participants on healthcare benefits and community resources.
