Job summary

Qualifications

• High school Diploma or GED required.
• Experience with Medi-Cal billing is highly desirable.
• Must obtain Community Health Worker state certification per policy.
• Driving record must meet guidelines and have at least 3 years of driving experience.

Responsibilities

• Provide health education to promote Medi-Cal member health and address barriers.
• Assist with health navigation, referrals, and connecting members to resources.
• Engage in-person outreach and accompany clients to appointments.
• Support wellness planning and independent living skills.
• Maintain PHI confidentiality and complete required forms timely.
