Job summary

Beacon Community Connections seeks a Reentry Community Health Worker to assist formerly incarcerated clients with social needs, connecting them to resources and healthcare, including behavioral health. You will advocate for access to services, coordinate with parole/probation staff, and help participants self-navigate complex care systems.

Responsibilities include conducting follow-ups, coordinating referrals, and maintaining client records in an electronic health record.

Qualifications

• High school diploma or equivalent is required.
• Bachelor's degree in social work, sociology or related field preferred.
• Demonstrated ability to communicate effectively in writing and verbally.
• Ability to document client activities accurately in an electronic health record.

Responsibilities

• Provide social care services to former incarcerated clients through assessment and navigation.
• Advocate for clients to access services and coordinate with community partners.
• Maintain relationships with Hub Partners and perform follow-ups.
• Conduct monthly case staffing and telecommunication with relevant agencies.
• Input information into an automated database and maintain accurate records.
