Qualifications

Responsibilities

• Conduct in-person and telephone outreach to Medi-Cal members receiving ECM.
• Build trusting relationships and help members identify goals and next steps.
• Provide post-release engagement and reentry support.
• Complete warm handoffs for members transitioning to ECM services.
• Document care plans and update follow-up tasks.
