Family Health Centers of San Diego is seeking a Patient Health Navigator / Certified Phlebotomy Technician to provide direct and indirect patient services, including referrals, appointment coordination, and coordination with multidisciplinary teams.

The role requires venipuncture skills, adherence to documentation standards, and collaboration to improve retention in care for medically underserved populations.

Qualifications

• High School diploma or GED required.
• CA Phlebotomy Technician license or certification preferred/required.
• Two years of experience in health education or medical field preferred.

Responsibilities

• Serve as liaison between patients and care teams to improve access to care.
• Perform venipuncture, finger stick, or capillary blood collection per protocol.
• Input and maintain accurate documentation and test results in the system.
