Job summary

Voluntary Protection Programs Participants' Association, Inc. is seeking a Community Health Worker/Patient Navigator to support the Refugee Program Team.

The role focuses on outreach, patient engagement, care coordination, and education to promote quality patient care and better health outcomes within the community. The CHW will identify and recruit patients, assist with care plans, coordinate referrals using Healthify and related tools, and advocate for patients on transportation,

Qualifications

• Two years of relevant work or college-level internship experience.
• Bilingual requirement: English plus another language.
• CHW Core Training within first year of employment.

Responsibilities

• Outreach to identify, recruit, and support patients to engage in care and care management services.
• Provide patient education and advocacy to assist patients with accessing services.
• Document services as required and assist in meeting program requirements.
• Collect and analyze data related to the project with data management staff.
• Facilitate referrals for patients using Healthify, E-referral, Community Help and other tools.
• Advocate for patients regarding transportation, interpretation, and health insurance.
• Maintain strong organizational and communication presence across departments.
• Perform other duties as assigned.
