Job summary

Family Health Center of Worcester, Inc. seeks a Community Health Worker to assist Primary Care Teams by identifying, recruiting, and supporting patients in care and care management services; you will educate and advocate to help patients access services and follow through on referrals as part of the population health team.

Qualifications

• Bilingual in English and another language relevant to patient population.
• CHW Core Training within first year of employment.
• Minimum of two years relevant work or internship experience; able to interview patients and coordinate with services.

Responsibilities

• Supports the care team with patient follow-up, reminders, and addresses barriers to care.
• Documents services and assists in meeting program requirements.
• Collects and analyzes data related to the project with data management staff.
• Provides outreach and explains services; coordinates treatment plans for patients.
• Facilitates referrals using Healthify, E-referral, Community Help, and other tools.
• Advocates for patients regarding transportation, insurance, and FHCW services.
• Acts as a hub of information across multiple departments; performs other duties as assigned.
