Job summary

COPE Health Solutions is seeking a Community Health Worker to help patients and families access community services, navigate the health care system, and adopt healthy behaviors. The CHW supports care managers and providers to promote comprehensive, coordinated care.

Tasks include enrollment assistance, outreach, and follow-up, with an emphasis on cultural and linguistic responsiveness, and maintaining HIPAA compliance while coordinating with the care team.

Qualifications

• Experience educating members about ECM services and enrollment
• Ability to navigate health care system and promote self-care
• Strong communication and trust-building with patients and families
• HIPAA/compliance awareness in daily tasks
• Willingness to support care team and coordinate with providers

Responsibilities

• Educate members about ECM services and enrollment.
• Assist members in navigating the health care system and improving self-care.
• Establish trusting relationships with patients and families; provide support and encouragement.
• Conduct follow-up and basic motivational interviewing with patients/families.
• Bridge conversations and conduct outreach to schedule appointments and close care gaps.
