Family Health Center is seeking a Community Health Worker/Patient Navigator to support the Refugee Program Team in Worcester, MA. The CHW will outreach, recruit, and assist patients to engage in care and care management services, and provide education and advocacy to access services.

The role involves coordinating referrals, assisting with transportation and insurance issues, and serving as a hub of information across departments.

Qualifikationen

• High school diploma or GED required; Bachelor’s degree preferred.
• Bilingual in English and another language appropriate to FHCW patient population.
• Community Health Worker Core Training required within first year of employment.
• Minimum of two years relevant work or college-level internship experience.
• Certificate of completion for Community Health Worker or Medical Interpreter preferred.

Aufgaben

• Outreach, identify, recruit, and support patients to engage in care and care management services.
• Provide patient education and advocacy to access services and follow referrals.
• Assist care team with patient follow-up and reminders.
• Document all services and assist in meeting program requirements.
• Collect and analyze data for the project and collaborate with data management staff.
• Coordinate treatment plans and referrals using Healthify, E-referral, and Community Help.
• Advocate for patients on transportation, insurance, and services across departments.
