Job summary

Qualifications

• Two years of relevant work or college-level internship experience required.
• Bilingual in English and another language appropriate to the patient population is required.
• Community Health Worker Core Training is required within the first year of employment.
• Bachelor’s degree preferred for this role.

Responsibilities

• Outreach to identify, recruit, and support patients to engage in care or care management services.
• Provide patient education and advocacy to assist patients in accessing services.
• Document services and assist in meeting program requirements.
• Collect and analyze data related to the project with data management staff.
• Coordinate treatment plans and facilitate referrals using Healthify, E-referral, Community Help, and similar tools.
• Advocate for patients regarding transportation, interpretation, health insurance, and FHCW services.
• Serve as a hub of information across multiple departments with strong organizational and communication skills.
