Job summary

Qualifications

• Preferred CHW certification completed or in progress.
• Additional education in healthcare, human services, social work, or a related field is preferred.
• Experience in a physician health care office or similar setting, preferred.
• Clear oral expression, listening, and writing; proficient with EMR systems.

Responsibilities

• Connects clients with healthcare, social services, and community resources.
• Provides health education and information in a clear, culturally appropriate manner.
• Assists clients in applying for healthcare, food, housing, transportation, and other programs.
• Follows up with clients to assess needs and encourage service utilization.
• Maintains accurate and confidential client records.
• Collaborates with healthcare providers, social service agencies, and community organizations.
