Job summary

MultiCare Health System is hiring a Community Health Worker to join an interdisciplinary care team and assist in care coordination, informing patients about community resources, and improving population health.

Qualifications

• High School Diploma or equivalent is required.
• 2 years of health care related experience is required.
• Experience in utilization management, community resources, and/or financial and quality issues affecting patient care is preferred.

Responsibilities

• Performs cross continuum care activities, which may include coordinating care between primary care clinics, specialty care clinics, urgent care clinics, or other sites of care, as appropriate.
• Engages in outbound communication to patients and keep them engaged in their care, set-up appointments, and prompt them to act.
• Helps patients access community resources which may include scheduling appointments.
• Establishes contact with community partners, travel is required.
• Develop tools to educate peers on patient advocacy and community resources.
• Participates in community health fairs and events.
• Educates the care team on available community resources.
• Participates in quality improvement activities.
