Job summary

Ingham County Health Department’s Maternal Child Health Division seeks a Community Health Worker to serve as a bridge between underserved communities and health and social services. Through home visiting, you will help clients overcome barriers to health, education, employment, and resources.

You will assist with forms, coordinate appointments, and develop resource guides while conducting outreach and maintaining documentation to ensure continuity of care.

Qualifications

• High School diploma or equivalent required; MiCHWA curriculum post-hiring preferred.
• Experience in social services, public health systems, or maternal child health services is preferred.
• Willingness to travel throughout the County and work with diverse communities.

Responsibilities

• Assist clients with filling out forms and coordinate appointments with providers.
• Create, maintain and distribute resource guides for food, housing, and services for the community and partners.
• Document client interactions in the program’s system and follow care coordination protocols.
• Assess social determinants of health (SDOH) and provide referrals based on needs assessments.
• Assist in Medicaid, SER, and food stamps applications and navigate DHHS case workers.
• Provide Medicaid outreach and education and coordinate transportation and referrals.
