Job summary

Honor Community Health is hiring a Full Community Health Worker to join the Care Management team in Oakland County. The CHW will support six core services and work with patients and families to address complex physical and behavioral health needs.

Responsibilities include tracking, follow-up, and documenting care in EHR and population health tools, with emphasis on patient-centered, accessible care. We seek compassionate, organized individuals dedicated to serving communities in Michigan.

Qualifications

• Two years of related experience or lived experience in the communities served.
• Current MiCHWA Community Health Worker certification or ability to obtain within 12 months of hire.
• Strong computer skills: Microsoft Office, EPIC, CHAMPS.
• High school diploma or equivalent required.

Responsibilities

• Deliver integrated care through six core CHW services: care management, coordination, health promotion, transitional care, family support, and referrals.
• Document care details in the Electronic Health Record and population health tools.
• Communicate effectively, stay organized, manage time efficiently, and multi-task in a fast-paced environment.
