Job summary

Qualifications

• High school graduate or equivalent required.
• Customer service experience required.
• Ability to work with a multidisciplinary healthcare team and the public.
• Basic computer skills required.
• Must be able to sit, stand, walk, bend and lift for extended periods.

Responsibilities

• Organizes workflow and appointments by coordinating with Care Managers and leadership to ensure health risk assessment data is collected and follow-ups are scheduled.
• Answer patient calls, field questions, and assist patients in making care manager appointments.
• Schedules appointments and follow-up appointments via telephone for care management patients.
• Reschedule or cancel appointments for care management patients; maintain confidentiality and HIPAA compliance.
• Maintain care manager schedules by making proper appointments and collecting required data.
• Participate in agency-sponsored staff training events.
• Protect patient confidentiality and adhere to HIPAA policies; provide guidance on clinic policies as needed.
