Job summary

Family & Children’s Services in Oklahoma City seeks a Case Manager to coordinate and deliver client-centered care across homes, communities, and facilities. The role requires collaboration with families and professionals and ongoing case reviews, documentation, and education to clients.

Qualifications

• Must be a high school graduate or have HS equivalent.
• Bachelor's degree preferred.
• Experience in mental health, criminal justice, or community-based social services required.
• Valid Driver's License required to travel to locations as needed.

Responsibilities

• Conduct comprehensive assessments of clients' physical, psychological, social, and environmental needs.
• Implement individualized care plans in collaboration with clients, families, and other stakeholders.
• Coordinate and monitor the delivery of services to ensure timely and appropriate care.
• Advocate for clients' rights and assist them in accessing necessary resources and services.
• Provide emotional and practical support to clients and their families.
• Maintain accurate and up-to-date documentation of clients' care, progress, and outcomes.
• Facilitate communication among multidisciplinary teams including healthcare providers, social workers, and therapists.
• Participate in case conferences, team meetings, and other collaborative activities to ensure coordinated care.
