Job summary

Roots Community Health Center seeks a Health Navigator to be the primary care manager for individuals and families. You will introduce clients to services and maintain ongoing contact with each participant, supporting enrollment in public benefits and coordinating with clinic staff.

The role requires on-site work, strong organizational skills, and a compassionate approach to diverse communities. Experience in nonprofit settings is preferred.

Qualifications

• Experience working in non-profit or community health settings preferred.
• Ability to engage diverse populations with sensitivity and respect.
• Proven ability to document and report accurately.
• Valid California Driver License expected (if required by role).

Responsibilities

• Develop rapport and relationships that foster trust for participants.
• Provide face-to-face visits and outreach calls based on needs.
• Assist enrollment in Medi-Cal, CalFresh, CalWorks, etc.
• Offer health coaching and glucometer training as needed.
• Support clinical staff in daily operations and complete documentation.
