Job summary

CHCCC is seeking a Navigator in the Behavioral Health Department to act as the bridge within the CHC clinical network throughout San Luis Obispo and Santa Barbara counties. The Navigator links patients into BH services, supports BH clinicians, and conducts case management to improve access and coordination of care.

Qualifications

• High school diploma or GED equivalent required.
• Associate’s degree in Human Services, Addiction Studies, Psychology, or related field OR minimum five (5) years of progressive front office medical and/or behavioral health experience may substitute Associate’s degree.
• Bilingual – ability to read, speak, and write in English and another language is preferred, not required.
• Experience with Microsoft Office, Google Suite, and EHR systems preferred.

Responsibilities

• Ensure BH charts are complete and verify insurance and payer status at every visit.
• Assist patients with forms with professionalism and advocacy for diverse needs.
• Inform patients about all CHC BH services and make referrals as needed.
• Provide quality customer service using AIDET standards.
• Handle high volume of patients and adapt to change.

Education

Associate’s degree in Human Services, Addiction Studies, Psychology, or related field
