Qualifications

• High School Diploma or GED required.
• Two years office and/or related experience preferred.

Responsibilities

• Assist the Care Manager in provision of services for the entire caseload.
• Update roster information and link patients to care.
• Educate patients on health conditions and provide appointment reminders.
• Maintain the care records including scanning documents into the EMR and office administration.
• Schedule appointments for the Care Management team.
