Job summary

Indiana University Health is seeking a seasoned leader to guide the Community Health Worker team across Gateway Plaza and offsite clinics. You will oversee daily operations, coach staff, and ensure high-quality service delivery to meet program goals and client needs.

The role requires 7–10 years of leadership experience, a High School Diploma or GED, and an Associate's Degree preferred. Strong Windows/MS Office proficiency is expected.

Qualifications

• High School Diploma or GED required; Associate's Degree preferred.
• Requires 7–10 years of relevant leadership experience.
• Familiarity with MS Office (Word, Excel, PowerPoint) and customer data software.

Responsibilities

• Provide day-to-day leadership, guidance, and support to the Community Health Worker team and departmental managers.
• Oversee daily operations, workflow, and service delivery to meet program goals and client needs.
• Train, mentor, and onboard CHWs, including developing training materials and continuing education opportunities.
• Coordinate with healthcare providers and community partners to ensure seamless service delivery.
• Monitor program outcomes through data collection, analysis, and reporting; identify opportunities for improvement.
