Cleveland Clinic Health System is seeking a Community Health Worker Workleader to oversee daily operations of CHWs, provide leadership, and coach teams to deliver a positive patient experience within the Pathway Hub.

The role includes guiding performance reviews, supporting Epic documentation, ensuring metric achievement, and training CHW peers in community health outreach at the Community Health and Wellness Center.

Formación

• High school diploma or GED.
• CHW Certification through the Board of Nursing.
• Valid Driver’s license.
• Four years of CHW or patient navigation experience.
• Ability to work under pressure and meet deadlines.
• Ability to communicate effectively with diverse backgrounds.

Responsabilidades

• Provide leadership and training to assigned Community Health Workers.
• Oversee daily operations of CHWs and support positive patient experience.
• Coach CHWs to work effectively within Pathway Hub and achieve metrics.
• Perform quality reviews of monthly OKRs and ensure EPIC documentation.
