Job summary

Qualifications

• High school diploma/GED.
• CHW Certification through the Board of Nursing.
• Valid Driver's license.
• Four years of work experience as a CHW or patient navigation field.
• Demonstrated time management skills with the ability to balance multiple priorities and meet timelines.
• Ability to work effectively in stressful, sensitive and emergency situations.
• Ability to communicate effectively with individuals of varying backgrounds.
• Ability to consider cultural, social and behavioral factors in public health services.

Responsibilities

• Provide leadership and training to assigned CHW employees.
• Oversee daily operations of CHWs and coach them to deliver a positive patient experience.
• Support achievement of metrics and completion of EPIC documentation.
• Perform quality reviews of OKRs and ensure timely completion.
• Engage with community and health services to connect clients to needed resources.
