Job summary

Reema Health, Inc. seeks a Community Health Navigator (Community Guide) to build genuine relationships with members and guide them toward greater engagement in their healthcare. This remote/field role involves visiting communities, scheduling visits, and documenting work daily through Reema's technology platform.

You will work 50 years? with start-up pace, evolving roles, and collaboration with a multidisciplinary team to address social barriers and improve wellbeing.

Qualifications

• 18 years of age or older.
• Available to work Monday–Friday during core business hours.
• [Field-based] Valid driver's license, insurance, and reliable transportation
• Offers contingent upon passing a pre-employment drug screening and background check
• Comply with HIPAA and patient confidentiality standards

Responsibilities

• Conduct phone and in-person outreach and enroll members in Reema's care model
• Build and manage a caseload (80-120 members) and provide coverage during absences
• Assess member needs to build a member-centered care plan
• Establish trusting relationships and follow through in a timely, appropriate manner
• Help members understand why health activities matter and address hesitations
• Coordinate referrals and schedule appointments
• Independently organize and prioritize workload to hit monthly goals
• Engage with members via phone, text, email, and in-person visits
• Connect members to community and health resources
• Document all member work in the Reema App and maintain care plans
• Maintain HIPAA confidentiality
