Job summary

Corewell Health seeks a peer support paraprofessional to improve life skills and health outcomes for individuals and families. You will conduct home visits, educate on wellness and self-management, and help access community resources.

The role requires cultural competency, reliable transportation, and driving to appointments. Responsibilities include coordinating group activities, documenting progress, and collaborating with program staff to support clients and families in achieving their health

Qualifications

• High School Diploma or equivalent and completion of paraprofessional CHW Training Program (Completion within 180 days from hire date)
• CRt-BLS (AHA/ARC) within 60–90 days depending on program
• LIC-Driver's License - STATE_MI
• Associates degree or equivalent with 3 years of relevant experience

Responsibilities

• Participates in community events and activities to promote health maintenance and disease prevention.
• Conducts home visits to determine client and family needs and develop care plans.
• Assists clients with arranging medical appointments and accessing community resources.
• Mentors, empowers and advocates for clients to increase independence and skills.
• Document services, assessments, plans and evaluations of goals and education.
