Job summary

Corewell Health seeks a Community Health Worker in Big Rapids, MI to provide peer support, wellness education, and help families access community resources. You will conduct home visits, teach self-management of chronic conditions, and coordinate care with a multidisciplinary team.

The role requires a high school diploma, CHW training, BLS, and a valid Michigan driver’s license. This is a full-time, 40-hour week position, with local outreach and program development opportunities.

Qualifications

• High School Diploma or equivalent and successful completion of the paraprofessional CHW Training Program (Completion within 180 days from hire date).
• CRT-BLS certifications within 60–90 days.
• LIC-Driver's License - STATE_MI upon hire.
• CRT-Auto Insurance - upon hire.
• Associate's Degree or equivalent preferred.
• 3 years of relevant experience preferred.
• Knowledge of community resources and social/health services.

Responsibilities

• Participates in community events to promote health maintenance and disease prevention.
• Conducts home visits to assess needs and develop care plans; follows up for effectiveness.
• Assists clients with scheduling medical appointments and accessing resources.
• Mentors and advocates for clients to increase independence and skills.
• Refers clients to appropriate service agencies for additional support.
• Communicates with families and professionals to coordinate services.
• Documents assessments, plans, and progress; tracks trends for program evaluation.
• Coordinates group meetings under supervision; gathers data for evaluation.
