Waymark is seeking a Community Health Worker to be the frontline presence in the community, connecting patients to social support, advocacy and navigation. You will work with a multidisciplinary care team to improve health outcomes for Medicaid populations and help patients access providers and resources.

The role involves in-person, home, and clinic visits, with a required clean driving record, a valid license, and travel across surrounding counties.

• Attend a 2-3 week paid training program.
