Job summary

Aspire Rural Health System is seeking a Community Health Care Worker to support patients in EDs and inpatient settings by addressing social determinants of health that affect access to care and outcomes. Qualifications

• Certification as CHW/CCHW or willingness to obtain within 3 months.
• Training available for right candidate.
• Experience in healthcare or population health preferred.
• Knowledge of social determinants of health and their impact on access.

Responsibilities

• Identify and address social determinants of health impacting access to care.
• Collaborate with ED staff, inpatient teams, social workers and community partners to connect patients with resources.
• Conduct patient outreach and follow-up; assist with Medicaid applications and benefits enrollment.
• Coordinate referrals to community resources and follow up on patient needs after discharge.
• Travel between Aspire locations as needed to support care transitions.
