Job summary

Qualifications

• BLS certification within 30 days of hire
• Valid driver’s license (ILSOS) required
• Experience in community outreach preferred

Responsibilities

• Perform client assessments at scheduled intervals and conduct Social Determinants of Health screenings
• Advocate for clients and help navigate health and social services
• Update care plans and promote adherence to improve outcomes
• Refer clients to needed health services and document interactions
• Build trusting relationships and assist with problem-solving barriers to care
