Aspire Rural Health System is hiring a Community Health Care Worker to support patients across Aspire locations. The role focuses on identifying social determinants of health that hinder care access and coordinating resources to improve care transitions and medication access.

The ideal candidate has CHW certification or willingness to obtain it, with healthcare or public health experience preferred. Travel between sites and collaboration with ED, inpatient teams, social workers, and community

Qualifications

• Certification as a Community Health Worker (CHW) or Certified Community Health Worker (CCHW), or willingness to obtain within three months of hire.
• We are also willing to train the right candidate.
• Experience in healthcare, hospitals, population health, public health, or community-based services preferred.
• Experience with patient outreach, care coordination, Medicaid applications, or benefits enrollment preferred.
• Knowledge of social determinants of health (SDOH) and their impact on healthcare access and patient outcomes.
• Strong communication, interpersonal, organizational, and problem-solving skills.
• Ability to work with individuals facing social, financial, housing, insurance, or healthcare access challenges.
• Knowledge of community resources and public assistance programs, or willingness to learn.
• Ability to work independently and collaborate with interdisciplinary healthcare teams.
• Basic computer skills and familiarity with electronic health records (EHR) preferred.
• Ability to maintain patient confidentiality and handle sensitive information professionally.
• Reliable transportation and willingness to travel between Aspire locations as needed.

Responsabilités

• Identify and address social determinants of health affecting access to care and transitions after discharge.
• Coordinate referrals to community resources and assist with Medicaid applications and benefit enrollment.
• Conduct patient outreach and follow-up calls to ensure services are utilized effectively.
• Travel between Aspire locations as needed to support patients and care teams.
