Job description

The Community Health Worker (CHW) works directly with patients, families, community partners, and interdisciplinary clinical teams to improve health outcomes, reduce barriers to care, and support equitable access to healthcare and social services. They serve as a trusted liaison between patients, healthcare providers, social service agencies, and community organizations, delivering culturally responsive health advocacy, health education, and health navigation services to Medicaid members with chronic conditions, unmet health-related social needs, or other qualifying needs. The CHW helps patients build self-management skills, navigate healthcare systems, maintain insurance coverage, address social determinants of health, and successfully connect to medical, behavioral, and community-based resources.

  • 1. Promote a safe, cooperative, and professional healthcare environment to ensure optimum patient care (as per HR Policy 113c).
  • 2. Adheres to all hospital policies, procedures, rules, and regulations, including but not limited to absenteeism, ETIME, cellular telephone usage, dress code, confidentiality, and rules of conduct.
  • 3. Makes Customer Service a priority, treating patients, visitors, community partners, and co-workers in a professional manner exercising courtesy, respect, and cultural humility.
  • 4. Demonstrates standards of performance and behaviors consistent with the DRIVE to Patient-Centered Excellence.
  • 5. Deliver Community Health Worker services as assigned, including but not limited to:
  • 6. Collaborate with physicians, nurses, social workers, care managers, financial counselors, and community-based organizations to coordinate services and improve continuity of care.
  • 7. Document all CHW encounters accurately in the electronic medical record in accordance with Medicaid documentation requirements.
  • 8. Maintain patient tracking systems, referral logs, outreach records, and quality reporting documentation.
  • 9. Participate in quality improvement activities, workflow development, and population health performance initiatives.
  • 10. Contribute to educational materials, workflows, patient communication campaigns, and program reporting.
  • 11. Attend scheduled department, division, supervision, and training meetings and participate as appropriate.
  • 12. Perform other duties at the discretion of leadership.
Requirements
  • Associate's degree required, Bachelor's degree preferred.
