They serve as a trusted liaison between patients, healthcare providers, social service agencies, and community organizations, delivering culturally responsive health advocacy, health education, and health navigation services to Medicaid members with chronic conditions, unmet health-related social needs, or other qualifying needs. The CHW helps patients build self-management skills, navigate healthcare systems, maintain insurance coverage, address social determinants of health, and successfully connect to medical, behavioral, and community-based resources.
