Job summary

Monarch Healthcare in North Carolina seeks a Community Specialist (PRN) to build positive relationships with individuals served and their families in work, home, school, or community settings. You will assess hopes, provide supports, and help individuals achieve personal goals while supporting caregivers.

Requirements include a High School Diploma and a valid USA driver’s license with experience working with adults with mental health diagnoses or developmental disabilities.

Qualifications

• High School Diploma is required.
• Valid driver’s license (USA) is required.

Responsibilities

• Assess each person receiving services for hopes, dreams and desires and provide supports that facilitate achievement.
• Ensure rights of individuals are protected and promoted; assist with advocacy and provide caregiver relief.
• Serve as a resource on community agencies, services, and supports to meet identified needs/goals.
• Supervise, educate and monitor individuals in work, home, school, or community settings as needed.
• Provide one-on-one support to meet emotional, physical, and medical needs.
• Maintain a safe environment by practicing safety procedures and documenting as required.
