Job summary

Monarch Healthcare in North Carolina is seeking a Community Specialist to build positive relationships with individuals receiving services and their families in work, home, school, or community settings. You will help them achieve personal dreams and provide periodic caregiver relief.

You will assess goals, protect rights, advocate, coordinate with community services, supervise and provide one-on-one support, and document progress while maintaining safety and compliance.

Qualifications

• High School Diploma is required.
• Experience in caring for adults with mental health diagnoses or developmental disabilities is preferred or required.

Responsibilities

• Assess individuals for hopes and goals and provide supports to help achieve them.
• Protect and promote rights; assist with advocacy and provide caregiver relief.
• Serve as a resource on community services and supports meeting identified needs.
