Job summary

Futures Unlimited, Inc. is hiring a Service Facilitator/Training Coach to support a designated caseload with on-site skill training and community work-site support. The role collaborates with the Employment Services Team to build community employer relationships and enhance job development efforts.

The position reports to the Director of Business Operations & Employment Services and involves daily documentation, training delivery, and liaison activities within the community.

Qualifications

• High School Diploma or GED.
• Minimum 1 year experience working with persons with disabilities.
• Valid Illinois Driver’s License and access to a vehicle during work hours.
• Strong organizational and customer service skills.
• Solid communication and interpersonal skills.
• Ability to process computer data and format/generate reports.
• Advanced Microsoft Office skills such as PowerPoint, Publisher, Excel, and Word.

Responsibilities

• Follow CARF, DRS, and DHS guidelines to develop, implement, and review program plans for each individual.
• Document progress and information in individual trainee files.
• Report billable hours monthly to maximize funding and assist in job placements.
• Document SEP and Community Contract Training and support services as required by funding agencies.
• Provide day-to-day skill training and support at community employment sites.
• Assist trainees with job applications, resumes, and interviews to strengthen search efforts.
• Coordinate services with other Employment Services Team members to match community needs.
