Job summary

Futures Unlimited, Inc in Pontiac, IL seeks a Service Facilitator/Training Coach to support a designated caseload with on-site skill training and community work-site support. This role also provides agency-based training and collaborates with the Employment Services Team to strengthen community employer relationships.

You will document progress, track billable hours, and ensure funding and regulatory compliance while guiding trainees through SEP and contract training.

Qualifications

• High School Diploma or GED.
• Minimum 1 year experience working with persons with disabilities.
• Must have valid Illinois Driver’s License and access to a vehicle during work hours.
• Strong organizational, and customer service skills.
• Solid communication and interpersonal skills.
• Ability to process computer data and format/generate reports.
• Advanced Microsoft Office skills such as PowerPoint, Publisher, Excel, and Word.

Responsibilities

• Follow CARF, DRS, and DHS guidelines to develop, implement, and review service plans for each trainee.
• Collect data and document trainee progress in individual files.
• Report monthly billable hours to maximize funding and support DRS job placements.
• Document SEP and Community Contract Training and hours per funding regulations.
• Provide on-site skill training and support to trainees at community sites.
• Advocate for trainees at worksites and help them address concerns.
• Encourage trainee choice and coordinate community integration services.
• Foster employee integration through natural supports at SEP sites.
