Futures Unlimited, Inc. is seeking a CDS Coordinator of Community Integration to coordinate, supervise and document community activities for the Community Day Services program.

You will schedule activities, coordinate with Transportation Services, supervise DSPs, and ensure personal care and wellness needs are addressed during community events. The role emphasizes effective communication, teamwork, and developing community relationships to support participant growth and independence, reporting

• Strong communication and interpersonal skills.
• Must be able to supervise and coordinate staff and volunteers.

Responsabilités

• Coordinate and communicate scheduling of community integration activities with CDS management and staff.
• Supervise direct support persons during community activities and events.
• Address personal care and wellness needs of program participants.
• Ensure attendance and accurate timekeeping and complete incident reports in a timely manner.
• Meet regularly with CDS Team to address participant outcomes and program functioning.
• Assist CDS Manager of Operations with orientation and training of new DSPs and new clients.
