Qualifications

• Clearances (Act 34, Act 33, Act 73) needed.

Responsibilities

• Help patients navigate community services and resources.
• Provide education and support to expectant and new mothers.
• Conduct intake interviews and enroll patients in programs.
• Assist with applications and registration forms.
• Provide ongoing follow-up and support for patients.
