Community Health Worker – Full Time – Day Shift – Pittsburgh
Join Allegheny Health Network as a Community Health Worker. The role focuses on helping patients navigate medical care, community resources, and develop healthy behaviors.
General Overview
This position supports patients in overcoming social, medical, and community challenges that affect health. You will collaborate with medical providers, coordinate care, encourage healthy habits, and conduct community outreach, including home visits.
Essential Responsibilities
- Meet patients in clinic or hospital settings and perform a needs assessment, gathering information from patients, families, and medical providers. Help patients set health goals by working cooperatively with medical providers, case management, and health plans. (25%)
- Establish trusted relationships with patients and families while providing encouragement and motivation to meet health goals. (20%)
- Conduct weekly follow‑up calls and home visits to connect patients with community agencies that address barriers such as homelessness, substance abuse, hunger, transportation, and child care. (20%)
- Document each encounter in detail according to protocols and guidelines. (10%)
- Communicate consistently with program supervisor/manager to evaluate patient/family status, provide progress reports, and discuss interventions. (10%)
- Assist patients with benefit applications (e.g., insurance, food stamps) and organize records, schedules, and prescriptions. (5%)
- Maintain professional relationships with patients, visitors, AHN staff, providers, and care team members, effectively communicating AHN’s mission. (5%)
- Stay knowledgeable about community resources relevant to patient needs and utilize the resource database. (5%)
- Perform other duties as assigned.
Qualifications – Minimum
- Associate Degree in Social Services or equivalent experience as a community leader, human services professional, or volunteer in high‑risk communities.
- Minimum two years of experience living in, volunteering in, and/or working within high‑risk communities; knowledge of local resources (housing, food, support groups, financial assistance).
- Experience in outreach, customer service, social service, or a related field.
- Excellent written and verbal communication skills; ability to follow written and oral instructions; sound judgment in applying professional services.
- Strong active listening, social perceptiveness, interpersonal, and organizational skills.
- Ability to collaborate with clinical professionals and medical providers as part of a care team; basic understanding of clinical/medical terminology.
- Valid Act 34 Criminal Background Clearance Certificate.
- Valid Act 33 Child Abuse Clearance Certificate.
- Valid Act 73 FBI Fingerprinting Criminal Background Clearance Certificate.
Preferred
- Valid driver’s license (depending on location).
