Mental Health Connecticut (MHC) is a statewide not for profit provider agency. We have an immediate full-time opening for a Community Diversion/Transition Nurse in Waterbury, with statewide travel.

This role coordinates diversion from higher levels of care and transitions back to the community. Requirements include a minimum of two years as a Registered Nurse (RN) in mental health or related fields, CT driver's license, CPR/First Aid, and ability to drive locally and statewide.

Qualifications

• Minimum two years RN experience in mental health, medical, or home health industries.

Responsibilities

• Diverting program participants from nursing homes and avoiding acute care hospitalizations for psychiatric reasons.
• Transitioning program participants living in nursing facilities back into the community.
• Providing outreach, education and engagement regarding community based options to program participants residing in nursing facilities.
