Job summary

Oaks Integrated Care in New Jersey is seeking a Mental Health Diversion Case Manager to join a community-based treatment program in Burlington County. This full-time role offers a 40-hour work week (Tuesday–Saturday) with specific shifts and on-call needs.

You will assess information related to the population, recognize symptom changes, coordinate with the clinical team and law enforcement partners, and connect participants with community resources. Travel across counties in NJ is required.

Qualifications

• Bachelor’s degree or equivalent professional experience.
• Knowledge of criminal justice system and treatment resources.
• Understanding of mental health treatment needs and resources.

Responsibilities

• Assist mental health diversion program participants in navigating the criminal justice system.
• Support participants in complying with court-ordered conditions and individualized treatment plans.
• Monitor participant engagement in treatment services and coordinate with the clinical team.
• Maintain regular contact with participants through in-person and community-based meetings.
• Provide evening and weekend availability to address gaps in service accessibility.
• Monitor medication compliance when prescribed.
• Maintain regular contact to promote engagement and stability.
• Engage with individuals in jail and other judicial facilities as needed.
• Attend interdisciplinary team meetings and collaborate with stakeholders.
• Travel throughout New Jersey and surrounding tri-state area as needed.
• Complete documentation and maintain electronic clinical records in a timely manner.
