Job summary

Mental Health Connecticut (MHC) has an immediate full-time opening for a Community Diversion/Transition Nurse at our Independence Center in Waterbury, CT. Travel statewide is required.

The Nurse will divert participants from higher levels of care and assist in transitions back to the community. Salary is $76,323 annually with a competitive benefits package including medical/dental, vision, life and long-term disability coverage, EAP, a 403b retirement plan with company match, and paid time off.

Qualifications

• A minimum two years of experience as a Registered Nurse, preferably in mental health, medical, or home health.
• Must possess and maintain Registered Nurse (RN) status.
• Valid CT driver's license with auto registration and insurance.
• Ability to obtain CPR/First Aid and related agency trainings.

Responsibilities

• Diverting program participants from nursing homes and avoiding acute care hospitalizations for psychiatric reasons.
• Transitioning program participants living in nursing facilities back into the community.
• Providing outreach, education and engagement regarding community-based options to participants in nursing facilities with mental health conditions.
• Educating clients on medications, illness, substance use, hygiene, interpersonal skills, social relationships, and health care.
• Providing consultation to professional staff regarding Behavioral Health options to nursing facility discharge teams.
• Liaising with families and significant others to provide psycho-education.
• Coordinating with DMHAS, LMHAs, and participants with DSS Money Follows the Person and Mental Health Waiver staff.
• Assessing level-of-care needs and identifying appropriate community-based options.
• Maintaining knowledge of community resources for participants with mental health/substance use needs.
• Willing to drive a personal vehicle locally and statewide.
