Care New England's Providence Center is seeking a motivated Registered Nurse for the ACT Engagement and Diversion Team to assess and support adults with co-occurring conditions and housing instability. The role emphasizes compassionate, team-based care in both office and community settings.

Qualifications include RN licensure in Rhode Island, an Associate’s degree (Bachelor’s preferred), and experience with mental health and substance use. A $5,000 retention bonus is available.

Qualifications

• RI licensure as Registered Nurse (RN).
• Experience with psychiatric/mental health and substance use preferred.
• Must have valid driver’s license and insured auto.

Responsibilities

• Perform nursing assessments and update care plans to improve health outcomes.
• Provide in-person and community-based services to a caseload per program standards.
• Triage, assess crisis needs, and coordinate individualized care plans.
• Administer medications with EMR documentation and six rights of administration.
• Educate clients and families on diagnoses, plans, and medications.
