Job summary

The CENTER for Human Services seeks a Continuum of Care Coordinator to organize, document, and oversee CoC services for homeless individuals and families across Central Illinois. You will lead outreach efforts, meetings, and funding applications while advocating for improved services and system changes.

Ideal candidates have experience with diverse populations, training in related areas, and strong collaboration and communication skills.

Qualifications

• Experience engaging with diverse populations including people in crisis and homelessness.
• Ongoing training in mental illness, substance abuse, racial and social justice, and homelessness.
• Valid driver’s license, reliable transportation, and liability insurance on a personal vehicle.
• Proficiency with Microsoft Office and Google applications.
• Ability to pass a criminal background check as required by policy/laws.
• Excellent interpersonal and communication skills; ability to prioritize multiple tasks and meet deadlines.
• Maintain professional boundaries with challenging populations and non-traditional work conditions.
• Bilingual English and Spanish preferred.

Responsibilities

• Coordinate organization, implementation, documentation, and maintenance of Continuum of Care services for homeless individuals and families.
• Accompany street outreach and client engagement teams.
• Organize monthly meetings and committees, identify gaps, develop solutions, and assist with funding applications and required reports.
• Advocate for Continuum of Care services and promote system changes within Central Illinois counties.
