Continuum of Care Coordinator (Full-Time)
The CENTER for Human Services, a leading provider of behavioral health care services, is seeking a Continuum of Care Coordinator.
Responsibilities:
The CoC Coordinator shall be responsible for the organization, implementation, documentation, and maintenance of Continuum of Care services for homeless individuals and families throughout Central Illinois, including:
- Accompanying street outreach and client engagement teams.
- Organizing monthly meetings and committees, providing leadership and support, identifying gaps in services, and assisting in
the development of solutions and resources, and assisting with funding applications and required reports. - The coordinator shall be an active community member, advocating and promoting Continuum of Care services and positive system changes within identified Central Illinois counties.
Qualifications:
We seek flexible, driven, passionate, open-minded, client-focused, and independent candidates who can identify and act on client needs at the individual and community level. Qualifications and attributes for candidates include the following:
- Experience engaging with and advocating for a diverse population, including people in crisis, people experiencing homelessness, and/or individuals with very low income. Relevant education may be considered as substitute experience.
- Evidence of ongoing training and education in related areas such as mental illness, substance abuse, racial and social justice, and/or homelessness.
- A valid driver’s license, reliable means of transportation, and liability insurance on personal
vehicle. - Proficiency in the use of Microsoft Office and Google applications, including Outlook, Excel, Word, PowerPoint, and Google Docs.
- Ability to pass a criminal background check as required by agency policy and/or applicable laws/regulations.
- Excellent interpersonal and communication skills and the ability to prioritize multiple tasks and meet frequent deadlines.
- Ability to maintain professional boundaries and engagement skills with a challenging population and in non-traditional work conditions.
- Ability to maintain professional composure during emotionally challenging situations and interactions with survivors of domestic violence, sexual assault, stalking, and trafficking.
- Bilingual (English and Spanish)candidates preferred.

Salary Range:
$48.00 - $58,000 annually
