Job summary

Thresholds-Inc is seeking a Service Coordinator for the Continuum of Care program in Chicago. You will coordinate referrals and linkages to clinically indicated services to support housing retention and eviction prevention, reporting on program outcomes, and improving access to care for participants.

The role works within the Quality department to ensure data quality, grant compliance, and collaboration with health providers, housing administration, and grants accounting teams.

Qualifications

• Bachelor’s degree in social work, psychology or closely related field required.
• Master's degree in social work, counseling or a closely related field preferred.
• Two (2) years of relevant work experience in mental health is required.

Responsibilities

• Supports the mental and physical health care of members on an assigned caseload.
• Coordinates care with member’s mental health, physical health, and/or substance use providers.
• Works closely with community resources and organizations to coordinate member services.
• Provides support and attempts to remove obstacles to outpatient care.
• Facilitates outreach, engagement, referrals, coordination, and follow up for health services.
