The Salvation Army in Jacksonville, FL is seeking a Full Time Case Management professional to provide social service assistance to eligible clients. This role includes interviewing clients, entering data into HMIS, and coordinating seasonal programs, while maintaining careful records and ensuring program compliance.

Qualifications

• High school diploma or GED required.
• Minimum two years of experience in social or public service.
• Valid Florida driver license preferred.

Responsabilités

• Interviews clients requesting financial assistance to determine eligibility based on program guidelines and restrictions; records client disposition and other pertinent information such as income, expenses, family and work history; completes client applications; photocopies pertinent documents such as income statements, utility bills, leases, etc.
• Contacts utility organizations and landlords to arrange and process financial assistance; produces check requests, vendor documents and sends to applicable parties upon approval.
• Prepares client records in a timely manner ensuring all information is accurate and up-to-date; obtains signatures of clients as needed; accurately enters client data into HMIS database and applicable spreadsheets; maintains up-to-date social service case history files on all clients; maintains the confidentiality of same.
• Serves as primary client contact and coordinator for seasonal programs including Camp Keystone, Smart Start Back to School Program, Thanksgiving Baskets, Senior Angel Tree program, etc.; makes client contact; receives and processes applications and ensures clients of these programs are served effectively.
